This privacy policy explains how Montier Health Langwarrin collects, uses, stores and discloses personal information, including health information, in accordance with Australian privacy legislation.

The clinic provides administrative and facility services to independent medical practitioners who consult at the clinic. Personal information is managed to support the provision of healthcare services by those practitioners.

Why and when your consent is necessary 

When you register as a patient, you provide consent for independent medical practitioners consulting at this clinic, and authorised administrative staff acting on their behalf, to access and use your personal information for the purpose of providing healthcare services.

Access to personal information is limited to individuals who require it for clinical or administrative purposes.

Where personal information is required for purposes outside the provision of healthcare services, additional consent will be obtained where required.

Why do we collect, use, hold and share your personal information? 

Personal information is collected and held to support the provision of healthcare services by independent medical practitioners consulting at the clinic.

It is also used for related administrative purposes, including billing, accreditation, practice management, quality improvement, staff training, and compliance with legal and regulatory requirements.

What personal information do we collect? 

The clinic may collect the following information:

  • Name, date of birth, address and contact details

  • Medical history, medications, allergies, immunisations and clinical information

  • Medicare number (for identification and billing purposes)

  • Healthcare identifiers

  • Health fund and concession details where relevant

Dealing with us anonymously 

Patients have the right to interact anonymously or under a pseudonym where lawful and practicable. This may not be possible where identification is required for safe provision of healthcare services.

How do we collect your personal information? 

Personal information may be collected:

  • When a patient registers at the clinic

  • During consultations with independent practitioners

  • From other healthcare providers involved in care

  • From hospitals, pathology and diagnostic services

  • From Medicare, health funds, or government agencies where required

  • Via website, telephone, SMS, email or online booking systems

Information may be shared with My Health Record where the patient has consented or where otherwise permitted.

AI Scribe Tools
Some independent practitioners consulting at this clinic may choose to use secure AI-assisted documentation tools to support clinical note-taking.

These tools assist in generating draft clinical notes during consultations, which are reviewed and finalised by the practitioner before being included in the medical record.

AI tools are used solely to support clinical documentation and are not used for marketing or non-clinical purposes. All information is handled in accordance with applicable privacy laws and clinical governance requirements.

When why and with whom do we share your personal information? 

Personal information may be disclosed:

  • To other healthcare providers involved in a patient’s care

  • To third-party service providers supporting administrative functions (e.g. IT, accreditation)

  • Where required or authorised by law

  • To reduce or prevent serious threats to health or safety

  • For statutory reporting requirements

  • During use of My Health Record or electronic prescribing systems

Information is only shared where necessary for clinical or administrative purposes.

We do not disclose personal information outside Australia unless permitted by law or with consent.

How do we store and protect your personal information? 

Your personal information may be stored at our practice in various forms. Paper, which is scanned into your personal file and the original placed into a secure document destruction bin for collection. Electronic records are stored on our server and monitored by our IT company for security purposes. Visual X-Ray downloads are accessed by the provider’s secure platform. 

Our practice stores all personal information securely. Local desktops are password protected and all staff and doctors have their own individual passwords. Personal data is stored securely within the software or SharePoint where different levels of access are allocated depending on job roles. 

How can you access and correct your personal information at our practice? 

You have the right to request access to, and correction of, your personal information. 

Our practice acknowledges patients may request access to their medical records. We require you to put this request in writing and submit in person or via email and our practice will respond within a reasonable time, generally within 7 days. We prefer to send a copy of your records via medical objects to your new practice, but should you require a paper copy the cost will be between $70-90 depending on size of your medical file. We can send via encrypted email at no cost if you provide consent to do so but this method is not recommended as security cannot be guaranteed. 

Our practice will take reasonable steps to correct your personal information where the information is not accurate or up to date. From time to time, we will ask you to verify that your personal information held by our practice is correct and current. You may also request that we correct or update your information, and you should make such requests in writing to langwarrin@montierhealth.com.au.

How can you lodge a privacy-related complaint, and how will the complaint be handled at our practice? 

We take complaints and concerns regarding privacy seriously. You should express any privacy concerns you may have in writing. We will then attempt to resolve it in accordance with our resolution procedure. Our Practice Manager will be in contact with you within 48 hours of receiving your complaint.

We employ industry-standard security measures to protect your personal information from unauthorised access, disclosure, alteration, and destruction. However, no data transmission or storage system can guarantee 100% security, and users should be aware of potential risks.
